The Smile Campaign Terms of Service

Sickle Cell Saturdays Free Dental Screening & Treatment Program
Smile Campaign & “Smile of the Year” Selection
1. Program Overview
The Potter’s Smile Inc. (“TPS”) provides free dental screenings and limited dental treatment services during designated Sickle Cell Saturdays events. These services are designed to support individuals diagnosed with Sickle Cell Disease (SCD) and Sickle Cell Trait (SCT), with a focus on improving oral health outcomes and education.
Participation in this program is voluntary and subject to the terms outlined below.
 
2. Eligibility
To receive services, participants must:
  • Be diagnosed with Sickle Cell Disease or Sickle Cell Trait (documentation may be required)
  • Complete all required registration and consent forms.
  • Be present at scheduled Sickle Cell Saturday events.
  • Meet any additional criteria set by TPS or partnering dental providers.
TPS reserves the right to determine eligibility at its sole discretion.
 
3. Scope of Services
Services may include, but are not limited to:
  • Oral health screenings
  • Preventative dental care (cleanings, fluoride treatments, etc.)
  • Basic dental procedures (as available through volunteer providers)
  • Referrals for advanced or specialized care
Important:
  • Services are provided based on availability of licensed dental professionals and resources.
  • TPS does not guarantee treatment beyond screening or initial care.
  • Emergency or complex procedures may require referral to external providers.
 
4. Consent for Treatment
Participants (or legal guardians for minors) must:
  • Provide informed consent prior to receiving any dental service.
  • Disclose relevant medical history, including SCD-related complications.
  • Understand that all procedures carry some level of risk.
TPS and its partners are not responsible for complications arising from undisclosed medical conditions.
 
5. No Cost Services
All screenings and approved treatments provided during Sickle Cell Saturdays are offered free of charge. However:
  • Participants are responsible for any external or follow-up care costs outside TPS services.
  • No monetary compensation or exchange is permitted for services received.
 
6. Smile Campaign Participation
Participants in Sickle Cell Saturdays may be eligible to participate in The Smile Campaign, an initiative that promotes oral health awareness and recognizes transformative dental care stories.
By participating, individuals agree to:
  • Allow TPS to document their dental journey (photos, videos, testimonials)
  • Grant permission for use of media for educational, promotional, and fundraising purposes
  • Maintain active communication and attendance throughout the program.
Participation in the Smile Campaign is voluntary, and separate consent may be required.
 
7. “Smile of the Year” – Full Dental Restoration Program
Selection Criteria
The “Smile of the Year” recipient, who may receive a full dental restoration, will be selected based on the following criteria:
  • Medical Need: Severity of dental condition, especially as it relates to SCD/SCT complications.
  • Commitment: Consistent attendance, follow-ups, and adherence to treatment recommendations
  • Impact Story: Personal story demonstrating resilience, advocacy, or community impact.
  • Oral Health Improvement Potential: Likelihood that treatment will significantly improve quality of life.
  • Program Engagement: Participation in Smile Campaign activities and outreach.
Additional Terms
  • Selection is made by a TPS review committee and partnering dental professionals.
  • Selection decisions are final and not subject to appeal.
  • Full dental restoration services are dependent on funding, sponsorships, and provider availability.
  • Timeline for treatment may vary.
 
8. Media Release
By participating in TPS programs, participants consent to:
  • Photography, video recording, and interviews
  • Use of their likeness for marketing, social media, grant reporting, and awareness campaigns
Participants may opt out in writing; however, this may affect eligibility for certain campaign features.
 
9. Liability Disclaimer
TPS, its staff, volunteers, sponsors, and partner providers:
  • Are not liable for any adverse outcomes related to dental procedures performed.
  • Do not replace ongoing medical or dental care providers.
  • Are not responsible for missed appointments, delays, or service limitations.
All services are provided “as is” and based on available resources.
 
10. Code of Conduct
Participants are expected to:
  • Treat staff, volunteers, and other participants with respect
  • Follow all event rules and safety guidelines.
  • Provide accurate and truthful information.
TPS reserves the right to deny or terminate services due to inappropriate behavior.
 
11. Privacy & Confidentiality
TPS is committed to protecting participant information. Personal and medical data will:
  • Be used solely for program administration and care coordination.
  • Not be shared without consent, except as required by law.
 
12. Modifications
TPS reserves the right to update or modify these Terms & Conditions at any time. Updated versions will be communicated during program registration or events.
 
13. Agreement
By participating in Sickle Cell Saturdays, the Smile Campaign, or the Smile of the Year program, participants acknowledge that they have read, understood, and agreed to these Terms & Conditions.

The Potter’s Smile is the only 501c3 in the United States and possibly the world, that addresses the dental issues associated with Sickle Cell Disease and the Sickle Cell Trait.  We are dedicated to dental education, dental research, and access to quality dental treatment.

The exact number of people living with Sickle Cell disease (SCD) or the Sickle Cell Trait (SCT) in the United States of America is unknown. Congress passed the National Sickle Cell Anemia Control Act in 1972, and a law passed in 1974 requiring every child born of color to evaluate SCD. Unfortunately, the states were slow to comply, and it was not until 2006 (32 years later) that ALL states were testing. Now we have generations of people living here in the USA that do not know their sickle cell status. SCD can cause complications such as kidney disease, high blood pressure, and stroke. 
People may have these complications and may not understand why. The culprit could be SCD! Take this simple survey. One of our goals at The Potters Smile is to survey, test and evaluate for SCD. We believe that the cure is in prevention. The more knowledge you have, the more preventive we can become.
